Why won't a field show up, even though it populates initially?

Hello,

I am struggling with a fillable field in a document. I type in a date in the date field and it populates, but when I download the file, it's blank.  All the other fields populate except that one (see attached file). Any ideas?

How does you add the date? The field is read only.

Hello Bernd,

I have attached a copy of the blank fillable form. All the fields accept my content, however when I go to save, the date field is empty, despite entering a date.
