Working with forms...

I am trying to set up a PDF with form fields for some die cut labels that are 18 up on a sheet.
I usually only do a couple of these at a time, so I am wondering if I can put a button in the open space to the right side of the label that will allow me to print only certain labels on the sheet? Maybe an option to supress print on all positions except ones that I've selected a 'print' button? Of course I wouldn't want this button to actually print, either.
I've enclosed a pic below.

You can place a large white button field on top of each section and set it to hidden. When the check-box is ticked you change its display (using a script) to "hidden, but printable", thereby blocking that section from appearing on the printed copy. The check-box itself can be set as "visible, but doesn't print" to achieve the same result.

I started this with a 1 up label created in InDesign, exported a PDF then placed that one position, 18 up in another InDesign doc, exported a PDF of that & then created the form(s).
I'm guessing I'm going to have to go from 1 up to 18 up without InDesign, as I'm guessing any form info would get lost at that point?
I don't know much about scripting....

Ok, I have got it, in a 1 up PDF.
None of the buttons on the right will print out.
The first one clears/resets the form. the second one displays all 6 fields on the left & the last one hides all the content.
Now, how do I get that 18 up on an 11 x 8.5 page?
