I have Acrobat Reader DC, and I am looking at a PDF file that has images. with Acrobat Reader DC, can I add Alt description with Acrobat Reader DC.

You can add a comment.

You can add a description as comment. Use Tools > Comments

I did that. but it's not allowing me to save it.

Nothing. it doesn't show an alt tag to the image.

You can add a comment. A comment is not an alt tag. Alt tags cannot be added with Reader.
