Adobe Acrobat Reader export to Adobe Sign

I have completed a document on Adobe Acrobat Reader and tried to upload it to Adobe Sign.  In doing so, it shows no information I have "filled and signed" from my original document in Adobe Read to Adobe Sign.  The document is completely blank when uploaded to Adobe Sign.  Is there a way to bypass this?

If possible, please share the screenshots showing the changes.
