Adobe Acrobat Reader is misplacing my form content

I'm filling out a legal form. So I click "Fill & Sign," add my signature, print my name and add the date, and when I go "Back to document" my signature, printed name and date get moved about 3 inches below the fields.

If I click "Fill & Sign" again they get moved back into place, but as soon as I go back to the document, they move again.

We apologize for the inconvenience caused, as per the description above, your signature and date is moving 3 inches below the fields when you sign a PDF document, Is that correct?

Please check for any pending updates of Adobe Reader from Help>Check for updates. You may also repair the installation files of Adobe Reader from Help>Repair installation(only for Windows).

You may also reset the preferences of Adobe Reader, to reset the preferences, please refer to How to reset Acrobat Preference settings to default.

Please try to sign the PDF file through a different browser like Chrome, or Firefox, or Safari on URL https://cloud.acrobat.com/fillsign with your Adobe ID and password and see if this brings any difference.

If possible, try to sign a different PDF file and check. If the issue persists, please share the following details:
