Recently encountered an issue where the mail to function open the default email application, populates the subject line with the file name but does not attach the form.
any ideas as to how I could resolve this?

As you have mentioned the file does not attach to the email, could you please confirm if there is any link added to that email.
Please share the screenshot of the email window or a small video of the issue occurring at your end.
