Hi , I've moved your post from the Photoshop forum to the Acrobat Reader forum where you are more likely to get help with your issue.

There are conflicting information in your text fields:

  • There is no V (value) entry. Thus, a viewer can assume them to be empty.
  • The field appearance streams do display some text. Thus, they may not be empty.
  • But these stream contents are not build as described in the PDF specification. So the streams should be ignored?

This can result in different PDF viewers displaying your PDF differently.

There also are other structural shortcomings in the form data in your PDF. Please re-read the PDF specification.

Are you using an external library for this, such as PDFBox?

Are you using an external library for this, such as PDFBox?

At first glance it does not look like PDFBox (or iText), at least not in a current version: The initial xref table is segmented (which is forbidden and causes issues in signing workflows), and as far as I know current PDFBox and iText versions don't do this error (iText used to do it in the very early 7.0.x versions; Aspose has been doing it for years but their usual Info entry is not in the file).

Also it is unlikely that the form API of any established PDF library was used to fill the form fields, they know to fill the V value. But maybe some custom code using lowlevel PDF manipulation methods.
