Online form to PDF

I have a fillable PDF and an online form. I want to take the user entered information from the form and put it into the PDF so the user can save the PDF. I want this to be an automated process. What program or server process do I need?

This would require a server-side application, or something like a PHP script. Those things are beyond the scope of these forums, though.
