I have tried everything I can think of, Reinstalled, change folder view options, deleted default settings, removed registry items, and to no avail I cannot resolve this issue.

Sorry to hear about the trouble and we appreciate that you have followed all those troubleshooting steps on your own.
Through the title "Save As dialog missing textbox" I might not got your requirement correctly, but if I guessed right you are willing to get the Save As layout with Navigation Pane?
Try these steps, it might help-
As per my OS there is an Organize folder option at upper left corner. The screenshot showed in your initial post had a similar icon in the middle of "Hide folder" and "Save" icon. Click on the arrow and select Layout>Navigation Pane
Hoping that it will help changing the layout as per your desire.
