I try to fulfill the PFD-form, but the text, which i type in one of fields, is copied automatically to all the fields. For instance, I enter 23.01.18 in the first field for date, and this text appears in all the fields from beginning to end: 23.01.18, 23.01.18,23.01.18 and so on. How to fix this problem?

That happens because the fields have the same name. There's nothing you can do about it.

Report this issue to the authors of the form and ask them to fix it.
