Unlinking checkboxes in Acrobat Reader DC

I'm filling out a form with several checkboxes with No, Yes, N/A options in dropdown list. When I check one of these options, all the other boxes change to the same. They appear to have been linked by the document author within this group of questions. How do I as the user work around this to select the various options independently?  When I hover the mouse over this group of questions, a resizable box appears which incorporates this group.

The document author needs to solve this. These may be checkboxes with the same name or they may be radio buttons with the same export value. The creator has to rename the fields in Acrobat Pro.

 

As a "workaround", can you use the Comments feature? It may be locked down.

Thanks for your help, Jane. I wasn't sure what the Comments feature was, so
got in touch with the business that required the form to be completed and
pointed out the issue. I was told it was a new form, so probably hadn't had
much client exposure yet, hence no-one had spotted the linked checkboxes.
In the end I printed the form, made the necessary change using Tippex,
scanned it and returned it by email. Back to the Dark Ages!
