How has someone random bee able to comment on a file I shared it contained sensitive information and a someone called guest 1 appeared an commented on it should I be worried 😟

Hope you are doing well. Thank you for writing in!

Have you, by any chance shared a public link to any of your documents over a public platform? This could be an only possible reason why any other person could get access to a document.

If not, I suggest checking on your app's shared section to see the files you have shared and stop sharing of the file.
