Mobile App Distribution License Request form gone?

Hi there,

I would like to reapply for a distribution license for my company.
Until now I used the form, but now I am always redirected to a login page instead of the form with the URL https://adobe.allegiancetech.com/cgi-bin/qwebcorporate.dll?idx=W8RNXE. I do not have login data for this. I have opened a ticket with InMoment and they say that the link is not their responsibility, but that Adobe is.

I ask for correction or description of how I can organize the license in the meantime.
