How can I create pre-written comments associated with fields in a fillable form?

Hope you are doing well. Per the description, you want to create fields with pre-written comments.

The feature is available on the desktop version of the application. 

When you create a form, use the option to create a drop-down list, go to the list's properties -> Options, and then add the comments to be selected.
