I have 4 fields (Form Number) (First Name) (Middle Name) and (Last Name). The fields are regular filable. I am trying to create a field at the top of the page that will list the all of the fields on the top of the page that will be filled based upon the user filling in the forms (Form Number) (First Name) (Middle Name) and (Last Name). For example if the user fills in the following:
Form Number - 234
First Name - John
Middle Name - Joe
Last Name - Doe
The top field will read (234 - John Joe Doe)
