DC Reader - XML parsing error (invalid token)

A colleague of mine filled-in dynamic PDF form, saved and sent to me. However there was probably some improper symbol used and now form does not open neither on my PC or my colleagues.

Thats is the pic from the last (I checked) version of Adobe Reader:

I tried to repair it. No success.

Other software such as FOXIT gives the following message:

Different PDF 2 text parsers did not help also...

I don't know how to attach files on this forum so here is link for external source containing my PDF EDIT [unknown link removed by moderator]
