Saving template in Acrobat Reader

I have saved a form as a template.  The form is partially filled out with my information.  I need to make many copies with only the dates changed.  I have saved just the form, saved the form with the pertinent information and neither way seems to work correctly.  When I go to the form all the input information is missing.  What am I doing wrong?  Or should I not save as a template?

I don’t know what you mean by “save as a template”. Please explain what you do. Also the exact version of software used. Thanks.
