Acrobat 9.0 pro. How to get chapter headings in PDF document into bookmarks?

Acrobat 9.0 pro. Received 367 page PDF with 5 pages of sub-chapter headings but no bookmarks making it almost useless. How to get chapter headings into bookmarks either (a) from the received PDF or (b) from the original document before it became a PDF?

From an existing PDF it's very difficult to generate such bookmarks.

From the file used to create the PDF it depends on the file format. If it

was a Word file and you use the PDFMaker plugin to generate the PDF file

then it's quite easy to do it.
