Acrobat Pro DC - unable to create digital signature field

Hello:

I am using a trial version of Acrobat Pro DC and preparing a form from an existing .pdf document. I am unable to find a way to create a digital signature field that can be "Click (to sign)".

Note: The existing .pdf has a functioning digital signature field but when I click "Prepare Form" and select that form, it crashes Adobe Acrobat Pro every time. I have to edit the .pdf first, remove the existing digital signature field, save the .pdf and then "Prepare Form" and select it with the signature required box checked.

I've read many knowledge base articles and tutorials that reference a digital signature icon in the tool bar but I do not have that icon available for selection. This is what I see:

When I select the available signature icon (envelope with x) this simply creates a text field, which when the document is opened in Adobe Reader, that field disappears when clicked.

I have tried this with Adobe Acrobat Pro on Windows 7 as well as currently on Mac OS 10.10.5 (Yosemite). Any help with resolving this issue would be greatly appreciated.

The problem is you selected the signature required check box. When you do that, it limits you to creating a form that's compatible with Adobe Sign, which does not allow digital signature fields. What you can do is select the More button in the right-hand panel and select "Revert to Acrobat Form" and you'll be able to add digital signature fields.

I honestly don't know. The person who created the form is no longer with the company. Is there a way I can check?
