Adding field to form for someone to upload a photo or document

I am trying to add a field to a fillable form so someone can upload a document or photo. 

Any help would be appreciated.

What do I use if I want them to upload a document or pdf to the form?

They need to use the Attach File commenting tool, then. You van use a script to create the comment for them, though.
