Buttons in PDF forms

I have added Reset, Print and Submit Form buttons to a PDF form.  I then go into properties and set the action for each button and save the form.  When I go back to recheck, the action has changed (ex.: Print a form has changed to Reset form).  The lower part of that screen hasn't changed, but I don't know if the top part affects the functionality.  What would be causing this to happen?  All the buttons are using the Mouse Up option ... could that be the problem?

Did you give each button a different name?

Can you share the file in question (via Google Drive, Dropbox, Adobe Cloud, etc.)?
