Check box (or radio button) checked only when yes. It's a checklist.

Hi,

I'm trying to create a checklist form, and want to check off the line item only when the requirement is obtained. It doesn't have to be in order. 

I'm not able to make the boxes act separately. Right now if I check one, it checks all of them.  Can someone help?

Give all the fields unique names.
