combining pdf and it duplicates

When combining pdf's it creates duplicates rather than merging the new pdf

I have version 9.0 for Windows.  The forms do have same fields but they are different forms and the portfolio in not a good option for what I am needing.

I was using he Adobe Acrobat IX and it worked wonderfully on these same forms.  Is this is a glitch in the new version?

Form fields with the same name will get the same content.

Combine the files as portfolio.
