Create fillable form with existing PDF

I have created a survey with Surveymonkey.  I have downloaded and saved the PDF to my desktop.  I am running Adobe Acrobat X Standard, version 10.1.16 on Window 7.  I am trying to make this pdf survey fillable to send out.  I cannot get the check boxes and radio buttons to activate using my Adobe program.  This is the only survey I have this issue with.  My other surveys I am able to make the check boxes and radio buttons active using my Adobe.  Surveymonkey tells me it is an Adobe issue.  I have uninstalled and re-installed my Adobe program, ran the updates and used the repair installation tool.  Nothing seems to correct the problem.  Any suggestions would be most appreciated.

"Make" is not very precise on your action. Are you trying to use the "Form Wizard" to "Auto Detect Form Fields"?

This method is very poor for check boxes and radio buttons unless they are very large and dark.

Have you tried to manually add the form fields?

By an chance has a password or other special feature been applied to the form?

I am trying to manually add the fields.

It is not password protected.

I have not added any other special features to it.
