Deactivate fields when box checked no

Hello,

I am trying to deactivate series of fields on a form when a box is checked no.  I'm a novice with Javascript and have been successful with coding formulas for calculations but I can't seem to figure this out.  Are you able to help?

By "deactivate" do you mean set as read-only? Hide? Reset? Something else?
