Entering a Zip Code Brings Up A Name

I have this created in Excel but would like to do it in Adobe.

Basically I'd like to create something where a person can enter a Zip Code in a given space.

The results will be a Name and Contact Information pops up.

If we understand correctly, you are trying to get the person's Name and Contact information filled up automatically when Zip Code is entered?

In a simple editing format, this wont be possible. If it would be a PDF form and then it might be achievable with a script. Where a certain script would run to fill the form field accordingly.

If I understand the theory correctly you click the drop down box, click the item in the drop down box you seek, and the reaction is a name populates elsewhere?

I am trying to do this with every zip code in 6 States I work.

That's a lot of items for someone to scan through.

Doesn't have to be a drop-down. It can be a text field.
