You need a server-side processing engine, which is what LiveCycle products are about. Similar stuff exists using PHP and the free GhostScript libraries. Either way, it's nothing you click together in 5 minutes. Both methods have specific requirements and either cost you extra money or at least lots of work.

It can actually be done very easily with Acrobat. Just select the option to create a PDF from a web page, enter the URL, and Acrobat will generate an Acroform that you can further edit if needed. The specific steps you take depend on which version of Acrobat you're using.
