Fillable Fields Template

Can a template be created to add the same fillable fields to different PDFs?

For example: I need to add specific fillable fields to a form for clients to fill out. The forms are the same, only the client information would change. I don't want to manually add the fillable fields to every client's form. I tried the auto fill feature but doing so selects spaces in the form that I don't want fillable. Any suggestions?

Are talking about seperate files or multiple copies of the form in one file?

If you place multiple copies of one form in one file each copy of the had to unique field names in each copy.

I am searching for something similar.  What I have are Chain of Custody's that need to be fillable for our website. Our site needs these documents to be forms with specific field names so we can fill them out on the site.

Currently I am opening up the new COC > converting it to pdf from word > opening in acrobat X Pro> letting acrobat detect all the fields > deleting all the fields > copy / paste fields from a previous finished form > then adjusting sizes and new fields accordingly.  I am sure there is a better way.

What I am hoping to find is a way to name the fields automatically according to our list of pre-determined field names. Or another fix to get me the end result that is not as tedious as my current procedure.  Any help would be appreciated.
