Form made with Acrobat does not behave correctly on a Mac but does on a Windows machine does

I started a form with Publisher and then used Adobe Acrobat to add boxes to make choices and export the cost of the choices and calculate the result.  The form works fine on my Windows machine but not on my Mac. I have tried Internet Explorer, Google Chrome, Firefox, and Safari.  Each has the same result.  I have no idea why?  Any suggestions are appreciated.  It is a registration form and can be found at http://spark.umkc.edu.  Click on Registration form to get to my form with the problem.

Dain't expect forms to work in a browser, each one has its own technology. Instruct your visitors to download and open in Reader specifically. The default Mac PDF viewer is worse than useless.
