I am having trouble with a form that I created in Acrobat being returned blank. I am using the form to simply gather information about requests for design and production of marketing material from various departments and do not need or want to collect that information for a data base. The form is completed in Acrobat Reader and then returned to me via email as an attachment. More often than not the form comes back to me blank even though the person submitting the form has saved it as a pdf before attaching it. How can I keep this from happening?

Are you 100% sure they are using Adobe Reader, and not opening it in the browser window, for example, or using some other application? If it is Reader, then what version is used?

I have instructed them to download the form, complete it (I am assuming they are using Acrobat Reader... maybe that's a mistake), save the file as a pdf to their hard drive and then send it back as an attachment.

This is a classic symptom when the form is opened and saved using the Preview application on a Mac. You can tell by opening the form in Acrobat and selecting: File > Properties > Description
and seeing what it shows for PDF Producer. If Preview corrupted the form, it will show something like: Mac OS X 10.6.5 Quartz PDF Content
If that's the case, you can use Acrobat to export the form data to a data file (FDF) and import it into the original blank form that you distributed.

The PDF Producer reads: Adobe PDF Library 10.0.1. This particular form was completed on a PC, but it doesn't seem to matter, I've had them returned blank from Macs as well.
