Help! I need a form submitted as a different name, then emailed as a PDF back to me

Here's the problem:

I have a form I've created. It works Great.

It has 6 fields that are required, including Full Name, and formula fields that total their order.

It all works great.

I need the following in a button or something like it:

SAVE the file as another name with their last name (or other identification)

SAVE the fields they've entered their order into the order form

SUBMIT the PDF saved form with information to me via email (I have a SUBMIT button completed)

The Problem:

When I test the form with Adobe Reader, it says the form data CANNOT be saved - that only a BLANK form will be saved. ARGH!!!!

Help please!!!! I need this yesterday!

It sounds like you're using an older version of Reader. This should not happen with Reader 11 or DC, and can be avoided with previous versions by Reader-enabling the form with Acrobat DC: FIle > Save As Other > Reader Extended PDF > Enable More Tools
