Help With Auto Populate

I have an existing PDF.

Want to change it to where I can fill-in name, date, etc. Then I need it to be able to auto populate.

Does Adobe have a software package that will allow me to do this?

Do you mean copy the value from one field to another? You can easily do it in Acrobat. Just give the two fields the same name and they'll have the same value automatically.
