Interactive images within a pdf

Greetings.

I've got a physical therapy office that is looking to make their intake forms fillable - that's the so simple part it isn't even funny.  Here's the problem:

On their intake forms, they have a drawing of a body and request the client circle the area of pain and indicate the type of pain in the area, x for sharp, + for tingling, etc.

Has anyone done this before?  Is it do-able?

They can use the various commenting tools to do it.

The commenting tools do not circle or indicate the areas on a body.
