Once again, I ask: Why doesn't "Create Link" tool work consistently?

I love the "Create link" tool when it works, a real time saver. But often it does not create the link tag and OBJR tag. Sometimes it works, sometimes it doesn't. Anybody have a clue?

Frustrates me too. Also - gives no option to create the alternate description in the Contents key as required by PDF/UA (ISO 14289).
