Did you fill the form with Adobe Acrobat or Acrobat Reader? Don't use Apple Preview.

So here is what I did... I created the form in Adobe Acrobat. I saved it and sent it out for my team to fill out. They filled it out on their desktop and sent it off to me. When I open it on my desktop it shows filled. When I open it on my phone it is empty. I feel like I did something wrong in the creation or maybe not.

For creating the form I used Adobe Acrobat. For reading the form I have the pdf reader on the iPhone. I have never had an issue with it before not sure why this is happening

You need to find out what application was used by the other members of the

team to fill it the form fields.

The issue is most likely caused by them not using Adobe software.

I wouldn't be so sure... Many times users do things you don't want or

expect them to do.

I suggest to verify it's the case instead of to assume it is so.

When you open it with your phone or tablet are you clicking on "open in" and then selecting reader?  or are you just opening it from the email?  Sometimes you need to take that extra step to make sure it's opening with the app you want it to.  I hope that helps
