Overlay a PDF with Form Fields onto another PDF

I have created an image that mimics our company's stamps that has check boxes for approved, approved as noted, revise and resubmit. I have this saved as a PDF that has the form fields for the check boxes and date. Is there any way that I can save this and then add it to any PDF document i get sent? I am using Adobe Acrobat Pro 9 on a Mac.

Is there a reason you don't want to simply create a dynamic stamp instead?

Nope! New to using Acrobat Pro, wasn't sure of my options. I will check that out.

I'm sorry, I misunderstood the question. I thought you already had stamps available for use in Acrobat. Unfortunately, creating dynamic stamps that prompt the user for input just before they are applied to the document can be a bit complicated, especially if using a custom dialog that you program with JavaScript.
