PDF form fields are vanishing with flash piece

Hi - This is Manish and working on single page interactive PDF, where I use video, flash piece and form elements. But when I click on submit form or view the PDF in full screen mode, most of the form fields are vanishing and when I remove the flash element from PDF. All are working fine. Is there any solution we can use flash with forms?
