What kind of "form"? A PDF form? If so, what application are you using to fill it in and print it?

Does it contain form fields that you filled in, or did you use the Commenting tools?

If you are using the commenting tool or the typewriter tool you need to print the PDF document and annotations/comments.

Have you tried to print as an image?

If that is indeed what you're doing you should also make sure the in the Print dialog, under "Comments & Forms", you select "Document and Markups".
