Scanned documents ONLY - auto-detect form fields found, but don't show?

Hi.  I'm having an issue creating forms but for scanned documents only.  Auto-Detect finds the fields, lists them to the right, but those fields don't show up or register in the document itself.  I can add form fields manually without issue.  Properties have the detected fields as "visible," yet they are still not.  Any suggestions?  Very annoying.

Don't count on the form detection wizard to work with scanned documents. Do it manually.

Thanks - typically, I wouldn't rely on the Wizard.  However, the Wizard worked perfectly up until Friday or so; this issue is recent / sudden, leading me to believe there is some toggle or setting I inadvertently set off.  Any ideas would be helpful.  MT

accident.
