Something invalids signature after signing

Dear All,

I have created pdf form using Livecycle Designer.

During latest revision I've made unidentified mistake resulting in invalid signature after signing.

I've uploaded form to Cloud - http://adobe.ly/1LkJNe5‌ and you can try repeat error with e.g. "Podpis QMR" signature field.‌‌

I hope Polish language of form does not create so much problems.
