The licensing form at the bottom of the "ADOBE PDF LIBRARY SDK" is broken

at the bottom where it says "The Adobe PDF Library SDK is available from Adobe through a license agreement to OEMs, ISV integrators, and enterprise IT developers on a case-by-case basis. The first step in the licensing the Adobe PDF Library is to fill out the information request form (PDF, 45K). "

The information request form does not let you select the state you're in, nor does the "next page" button work or the submit button. To be able to use the SDK I must submit this form first so I'm hoping I can either manually enter the information and email it or find a fixed form

Almost everyone licenses the PDF Library from Adobe's partner DataLogics instead. Nobody has reported on licensing directly from Adobe but I think it must have been mostly for big enterprise wanting to partner.  https://www.datalogics.com/products/pdf/pdflibrary/
