I would like to build a web application where a user would be presented with the ability to open a PDF file in Adobe Reader, fill in their name/title and date, and electronically sign the PDF. When complete, there should be a button where the resulting PDF with data/signature posts back to a HTTPS site (and ideally close the Reader window when successful). This would only be one PDF, and only a few users to start. I don't need to extract any data on the back end, I just want the signed PDF. What Adobe software do I need to have to be able to create this PDF to be filled/signed in all recent versions of Adobe Reader, and do I need whatever Reader Extensions is?

Would like to inform that you may use Acrobat Pro DC for creating such forms. You may apply a 'submit' button so that user will submit the form after filling all the fillable fields.Before sharing the the form you have to save the form with Reader Extended Feature so that Reader Program users may also fill the form.
