Why do the radio buttons no longer work in a file where they used to?

My colleague needs to edit some government forms at their request, which he has done before, but all of the form buttons (specifically the "modify" button) are no longer clickable. I feel him saving the form caused the buttons to lose their edit-ability.  General links in the form still work, and the signature is still clickable for editing...

I am unsure on how to make the buttons clickable again, and not sure if re-doing the form from scratch is a viable option for him.

He is working on Windows 10 - Acrobat XI (reader?), I am working on Mac El Capitan with Acrobat X Pro.

Any help would be appreciated.
