Why does the form I created automatically fill the row down?

I created a form with columns. People completing the form enter an x in one of the columns to indicate the status of the project (ahead, on target, behind, not known.) All of the fields are working fine except one. In this particular column "not known" it automatically fills all the columns with an "x". Any idea why and how I can fix it?

In that case make sure that your fields have a unique name. Fields with the same name will automatically have the same value.
