dynamic stamp with radio button

Hi everyone. 

 

I am having trouble with a dynamic stamp. 

It has a radio button with 4 choices.

When I load the stamp to pdf, only the first choice shows up when it is selected. 

If I pick the other 3 options, it does not check the boxes. 

 

Did anyone have this issue before? Can anyone help me?

I copy and pasted from the stamp  I have. 

I don't really know anything about Script... How can I make it easy to read? 

Could you maybe give me a direction? so I can reformat it?

It is my office stamp and I have no idea who created and since when they used this stamp.

I just want to fix the stamp so that I don't have to manually check the boxes every time after placing the stamp...

Are you interested in learning JavaScript?  Or did you just want a solution? Because your issue is more work that is suitable for this forum. PM me if you are interested in hiring a developer to do the work. 

 

For what it's worth, this code looks a great deal like the stamp code used by BlueBeam.  Do you use BlueBeam?

Some of us use BlueBeam and Some of us use Adobe. Maybe it originated from one of the BlueBeam users... 

I thought it could be a simple fix... but maybe it isn't. I will do some study on javascript.
