How to create an options box that comes up when a PDF file is opened

I'm having the worst time trying to find answers to this...


What I have is a PDF document that consists of several individual forms.  I want to set up the whole document so that when you open it, it immediately creates an options box that allows you to choose the form(s) you want, and when you click OK, it generates only those forms.  I know how to code the appearance of the options box, but I have no idea how or where to code it to actually make it work.  I think it's in the docReady event, but beyond that I'm stumped.

 

I'm working in LiveCycle, ver. 11.0.1.[...].  Thanks in advance.
