Possible to create form with this requirements?

My question is, is it possible to create a form with the following requirements?

1. Open PDF

2. The supervisor fills in fields.

3. The manager clicks on one of the departments, then the form should be forwarded to defined, stored e-mail addresses for processing (6 departments, on each departement is an email address - the choice of the departements with radio button or dropdown field).

4. Example, the email was send to the department secretariat. Yes, this is possible. And there are a few different way it could work. For example the easy way to so just create Forward buttons for each individual in the process. But it could also be programmable so the process only moves forward. The more complex, the more complex the scripting. Here's are two articles that show how the emails could be programmed
