Add a form as a cover page to a PDF document

Having a hard time figuring this out. I have a fillable form for people to review my documents. I'd like the fillable form to be the first page and then the pages that follow would be the manual I want them to review. So I want to combine a form with a PDF. Can anyone please explain the workaround? Thank you!
