Adding Checkbox to Prepopulated Form

I am having trouble adding a checkbox to a prepoluated form and maintaining the data.  Anyone know how to add a checkbox and still retain the initial data added to the form?

Use Tools> Prepare Form and add the checkbox.

Hi Kimberly,

Following the steps suggested by community expert, you can add the checkbox to the existing Form. However, you need to make sure that document properties allows you to edit the form.

If you are the owner of the form, then there shouldn't be any trouble. If not, then you may first Open PDF>right click Document Properties>Security> changing the document is allowed or not.
