Adobe Pro XI form loses features in Adobe Reader DC

I made a fillable form using Adobe Pro XI on my desktop (Windows 10). 

 

I sent the form to my laptop (Windows 10). I can open the form with Adobe Reader DC version 20

 

I can fill in the form, save it and close it. I can reopen the form and continue to fill the form. 

 

But after I insert my signature as a *png using "Sign document by typing or drawing a signature" at the top right of Adobe Reader DC, and save, the fillable areas are gone and I can no longer fill the form.  

 

Any help is greatly appreciated. Thank you.

Signing the document locks the form fields. First fill the form fields then sign the document.
